VHS Working Papers - Option 2

OPTION 2 - Print and Fill

Please follow these steps in order to obtain student working papers. Any questions can be sent directly to the student’s counselor or to Miss Cass at fkcass@vestal.k12.ny.us

Follow the instructions below:

 

Step 1: Print AT17 Application

Click on either of these links to get a file to print to your home printer. Fill out PART 1 of the form only, and be sure that the parent/guardian signature has been completed.

Step 2: Proof of physical

NY State requires proof of a physical within the last 12 months. If the school nurse or athletic department does not have a current physical on file, you need to:  
 
a. Take a picture of your physical and email that picture to fkcass@vestal.k12.ny.us (you may have to pick it up from your doctor’s office – they will not email me directly), OR

b. Have your doctor fax proof of a physical to 757-2306 OR

c. Print the form (attached below in two formats) and bring it to your doctor to fill out.

Step 3: Return the forms to Vestal High School Guidance Department

After you have filled out the application, you may:

1. Mail it to the school at: 
        Vestal High School Guidance Department
        Attn: Working Permits
        205 Woodlawn Dr
        Vestal, NY 13850
 
2. Take clear pictures of the application/physical forms and email them to Miss Cass.

 3.  Arrange to drop it off by calling 757-2214 (There are limited times to drop it off, so you 
      must call first).  

 

Step 4:

Once the AT-17 application and the physical have been verified, Miss Cass will complete the working card. This may take a few days if we are waiting for a physical. It will be mailed by USPS mail to the family’s address as indicated in the Google Form.
