A physical exam is required for all children entering this school system for the first time, and for all children entering Pre-Kindergarten, Kindergarten and Grades 1, 3, 5, 7, 9 and 11. Proof of this exam must be a signed physical appraisal form. (A physical exam performed within 12 months prior to the start of the school year is acceptable. All forms are due by October.) If such an examination is not provided, the school is required by law to provide the exam. We recommend that the parent or guardian make an appointment to have the family doctor complete the physical, as he or she will perform a more complete physical exam and will give the required immunizations. New York State Education Law requires that schools provide the following health screenings:
New York State Education Department Guidelines require all medications, including over-the-counter medications that are to be given at school, to have the following:
Medications must be delivered in their original containers to the school by the parent or guardian; students are not allowed to carry medications to and from school.
Students who have a written order from their physician stating that they are self-directed and instructed in the proper use of their medication may carry emergency medications such as an Epi-pen, inhaler or insulin.

New York State permits schools to request an oral health assessment at the same time that a health examination is required. This is optional. For your convenience, Dental Health Certificates for each schools' Health Office are linked below. Your dentist can fill this out at your child's next check-up. If your child had a dental check-up before starting school, ask your dentist or hygienist to fill out Section 2 of the Dental Health Certificate form and then return the completed form to the school nurse as soon as possible.
New York State Education Law requires that BMI screening be included as part of the student's health examination. Each year, a sample of school districts are selected to take part in a survey by the New York State Department of Health. No names or student information is sent, only a summary. However, you may choose to have your child's BMI information excluded from this survey. For your convenience, a BMI Opt-out Form is linked below.
